Quality Home Health Care For Seniors
If you need healthcare services but have limited mobility or difficulty leaving your home, our in-home senior care services can help. These services are typically covered by Medicaid and can be accessed with an order from your doctor or healthcare provider.
We provide a personalized care plan to prevent, or treat, illness or injury in the privacy and safety of your home. Our focus is to help improve health and safety, regain independence, and become as self sufficient as possible. We aim to help maintain current levels of function or work to slow physical and/or cognitive decline.

Our team is dedicated to providing professional and compassionate care. Communication with our patients, familes and their medical provider(s) is important to us so we can ensure early identification of physical or cognitive decline and initiate timely treatment.

THESE SERVICES ARE IDEAL FOR SOMEONE EXPERIENCING:
- New or recurring wounds/injuries
- Recent discharge from hospital or post-acute care
- New diagnosis or debilitating disease or medical condition
- Need for assistance with medication management (new medications or frequent dose changes)
Skilled Nursing Services
Our professional nurses focus on delivering personalized attention and clinical expertise. We strive to enhance comfort and promote faster recovery by providing continuity of care right at home. Our nurses provide education and management re: medication and a cute/chronic illness with focus on prevention. Our team is skilled to address healthcare needs after discharge from an acute care setting to monitor/treat symptoms, including wound care. When there is a decline in physical or cognitive health, a Discovery At Home nurse can work with you and your medical provider to implement appropriate care in a timely manner.
Our nurses are trained to care for patients under the Cognitive Assessment & Memory Program (CAMP) – a therapy / memory exercise program tailored to your needs.
Occupational Therapy
Our occupational therapists work closely with you to help develop strategies that enhance your ability to perform daily tasks and increase mobility. Therapy is tailored to seniors to assess and address age related challenges. Our trained therapists help to rebuild confidence and independence so you can live life to the fullest in the comfort and familiarity of your home.
Speech Therapy
Our speech therapists play a vital role in helping seniors maintain or regain their ability to communicate; in addition, they are trained and experienced to address swallowing disorders and cognitive impairment. At Discovery At Home, we know the importance of addressing speech and language challenges and memory concerns. Through personized and targeted therapies, our speech therapists aim to improve communication and memory skills resulting in improved confidence with daily interactions.
Physical Therapy
Our experienced physical therapists can assist with improving balance, strength, endurance, mobility and safety awareness; skilled in techniques that can promote healing and minimize pain after injury or surgery.
Our therapists will perform a home safety evaluation, providing recommendations for appropriate assistive devices or medical equipment to maintain safety and prevent injury.
Home Health Care
These services are provided to you free of charge through your traditional Medicare Insurance. Services are available with an order from a Primary Care Physician. Home Health Services are ideal for a person who is unable to leave the home without difficulty.

In House Outpatient Therapy
These services are ideal for those who are looking to improve their daily mobility and quality of life. Outpatient therapy is appropriate for individuals who are mobile enough to attend sessions outside their home, such as a therapy gym, to achieve their therapy goals. Services are available with an order from a Primary Care Provider.

Our therapists can work with patients to decrease pain, increase strength, improve balance, decrease fall risk and restore abilities to complete activities of daily living, such as bathing, dressing, and meal prep. Outpatient therapy is covered at 80% under Medicare Part B, and the remaining 20% can be covered by a secondary insurance.
Outpatient Therapy

Outpatient therapy can treat patients with similar medical conditions as those treated under home health services. Outpatient therapy is appropriate for individuals who are independent enough to attend sessions in our Assisted Living or Independent Living on-site therapy gyms. Outpatient therapy is ideal for those who are looking to improve their daily mobility and quality of life.

Our Personal Care Services provide the physical and emotional support needed to safely manage health conditions and daily activities comfortably at home. Our compassionate team offers flexible services and can be available 24/7. With Discovery At Home, Personal Care Services are competitively priced and may be covered by Long Term Care insurance.

Home Health Aides and Certified Nurses Aides are available 7 days a week up to 24 hours a day to help with activities after hospitalizations, illness or as needed by the client to help ensure their safety while maintaining independence. Prices are available upon request. Some costs may be covered by insurances.
